Appointment and Prescription Refill Request Page

For Appointments:
| This is for ROUTINE appointments only. If you are having problems, please call us to get you seen as soon as possible. |
| We can only make appointments on the web for established patients. We welcome new patients, but we would like to get additional information on the phone while making the appointment. |
| You will need to give us your name and contact information. Please indicate if you would prefer a reply by email for the confimation. |
| Enter the date requested in MM/DD/YYYY format |
| We will check for openings on that date, and contact you to confirm. |
For Prescription Refills:
| We can only refill prescriptions for established patients who are current. If you missed your appointment, please call us for the refill. |
| We can mail you the prescription, or you can pick it up in the office at no charge. If you need a prescription faxed in urgently, there is a $5 charge. |
